Membership Rules you agree to upon purchase:

• I understand that my Membership is non-refundable and non-transferable.
• I understand that my Membership is no longer valid after its expiration date.
• I understand that in order to cancel my Auto-Pay Membership, I need to send an email to info@myyoga.nl 2 weeks before my scheduled pay date and that all other forms of cancellation will not be accepted.
• I understand this agreement shall continue on a monthly basis.
• I understand that I am allowed to suspend my Membership 1 time per year for a maximum of 8 weeks and shall do so in writing to info@myyoga.nl 1 week prior to the requested suspension time.
• I understand that the normal price of this membership is €110 per month and that I will receive the special discount of €20 per month for as long as I stay on the contract.
• I understand that MyYoga has the right to increase the price of the contract by the CBS index 1 x per year in the month of January.
• I understand that reservations for class must be made online in advance of attending a class.
• I understand the late cancel policy; whereby if I cancel my class less than 4 hours before the class or do not show up in time (5 minutes BEFORE class starts) I will be charged the late fee of €5.
• I understand that if my monthly payment is rejected by my bank for insufficient funds or making a refund booking without prior written consent from MyYoga to do so I will be charged a processing fee of €20.
• I understand that my monthly payments will be deducted from the bank account (or credit card) for which I have given details. This payment will come from a “payment processor” which is not MyYoga but called Stripe Technology Europe Ltd. The name of MyYoga will be mentioned on the description of the payment.

Monthly AutoPay Membership Agreement

Description of Membership: this is an unlimited monthly membership allowing you full use of our facilities and unlimited access up to any regularly scheduled classes at MyYoga. This membership shall commence on the selected starting date and shall continue on a monthly basis.

Frequency of Payments will be automatically charged monthly to the credit card or bank account (IBAN) you provide below on the day the agreement commences. Months are billed in advance. No prior notice will be given. You will receive and email informing you when the charge has been made.

Automatic Recurring Payment Failure: If your automatic recurring payment is declined, for insufficient funds or by making a refund booking without prior written consent from MyYoga to do so you will be charged a €20 processing fee.

Cancellation of Automatic Recurring Payment: You may cancel your membership at any time with a written notice by email info@myyoga.nl at least 2 weeks prior to the next payment date. The special membership price is good for as long as you remain on the program. NOTE: Making a refund booking via your bank does not constitute cancelling your membership.

Continuation of Automatic Recurring Payment: The monthly payments will continue for as long as you stay on the membership. Only by following the cancellation rules will it cease.

Changing Automatic Recurring Payment Method If you would like to change your Automatic Recurring Payment, including, for example, making a change from one credit/debit card that was initially selected to another credit/debit card, you must authorize the recurring payment with the new credit/debit card by notifying MyYoga in writing. Your new information will be implemented before your next billing cycle for your membership.

Suspension of Automatic Recurring Payment Suspension of your Automatic Recurring Payment will be allowed for medical reasons which preclude you from attending class for a minimum of one month or longer, which must be supported by a note from a physician showing date of injury/illness and time not allowed to practice the physical activities we offer. Your membership will be suspended for the length of time specified, at which time your membership will be reactivated. This Agreement is non-transferable and non-refundable.

AGREEING TO THESE CONDITIONS

By providing my credit card / bank details (IBAN) information (“Payment Method”), I AGREE that I have read and understand this Automatic Recurring Payment Agreement. In addition, I authorize MyYoga to charge €90 once every month to the specified Payment Method; and I authorize the financial institution for the Payment Method, specified below to charge my account and remit payment for membership to MyYoga. This authority will remain in effect until I give written notification, as required under this Agreement to terminate this authorization. I agree to the general terms and conditions as stated on the website www.myyoga.nl. Note: credit card information is stored in a secure format on MindbodyOnline, whereby employees of MyYoga do not have full access to all numbers and details of your credit card and therefor cannot authorize payment for anything further than what you have agreed to here.
